Epiphany Lutheran Church is a community who RAISES UP all of our CHILDREN and ADOLESCENTS to be healthy, hopeful, and faith-filled servant leaders in the world.

Medical release forms should be on file before youth attend off-site or overnight events at Epiphany.

Activity consent forms are required for each off-site and/or overnight activity for Epiphany children and youth.

 

Epiphany Lutheran Church intends to provide a safe and secure environment for all those entrusted to our care. We do this to encourage children, youth and their families to grow in their relationship with God and with one another.

Epiphany's Child Protection Policy applies to all adult and teenage servants (paid and nonpaid) who will have responsibility for supervising the activities of children, youth, and mentally challenged persons.

Sunday, March 24, 2 PM

CIA youth will be jumping high on March 24th, when they visit Sky Zone for an hour of jumping, followed by a snack and drink. Cost of this fun adventure is $18, plus money for snacks. We'll leave immediately following the Discipleship Formation session and return to Epiphany's parking lot by 5 PM.

Two consent forms must be completed! Go to the Sky Zone website to complete the "Before You Jump" online waiver. Send an email to Paula Atkins once you have completed the waiver.

Additionally, parents must complete Epiphany's Activity Consent Form; bring this form with you on March 24th.
